Medicare 101: The Essential Guide to Parts A, B, C, D & Medigap

1. The Four Fundamental Parts of Medicare

Medicare is the federal health insurance program primarily for individuals age 65 or older, and younger people with certain permanent disabilities. It is broken into four distinct components:

Part A (Hospital Insurance)

Covers inpatient hospital care, skilled nursing facilities, hospice care, and some home health care. Most Americans pay $0 premium if they or their spouse paid Medicare payroll taxes for 10+ years (40 quarters).

Part B (Medical Insurance)

Covers doctor visits, outpatient medical services, physical therapy, diagnostic testing, and preventive screenings. Requires a monthly standard premium set each year by the federal government.

Part C (Medicare Advantage)

An 'all-in-one' alternative approved by Medicare and managed by private insurance companies. Advantage plans bundle Parts A and B, usually include Part D prescription drug coverage, and offer dental, vision, and hearing benefits.

Part D (Prescription Drug Plans)

Standalone outpatient prescription drug coverage offered through private insurance carriers approved by Medicare to lower the cost of brand-name and generic medications.

2. Medicare Supplement (Medigap) vs Medicare Advantage

When enrolling in Medicare, one of your primary decisions is choosing between a Medicare Supplement (Medigap) or a Medicare Advantage Plan.

Medigap (Plans G and N): Medigap policies pay the 20% coinsurance and hospital deductibles that Original Medicare does not cover. You have the ultimate freedom to see any doctor or visit any hospital in the United States that accepts Medicare—no network hurdles or referral restrictions.

Medicare Advantage: Uses network-based care (HMO or PPO). Many plans have $0 monthly premiums and low predictable copays, plus valuable added benefits like dental cleanings, dentures, eyeglasses, and fitness gym memberships.

3. Crucial Medicare Enrollment Periods & Dates

Initial Enrollment Period (IEP)

A 7-month window: starts 3 months before your 65th birthday month, includes your birth month, and lasts for 3 months after. This is the optimal time to sign up for Part A and Part B.

Annual Enrollment Period (AEP): Oct 15 – Dec 7

Each fall, you can switch between Medicare Advantage plans, return to Original Medicare, or change your Part D drug plan for coverage effective January 1.

Medicare Advantage Open Enrollment (OEP): Jan 1 – Mar 31

If you are already enrolled in a Medicare Advantage plan, you can make a one-time switch to another Advantage plan or return to Original Medicare with a Part D drug plan.

4. Understanding and Avoiding Costly Lifetime Penalties

Failing to enroll in Medicare when you are first eligible can trigger permanent, lifelong financial penalties:

Part B Late Penalty

Adds 10% to your monthly Part B premium for each full 12-month period you were eligible but didn't enroll. You pay this surcharge for as long as you have Medicare.

Part D Late Penalty

If you go 63 consecutive days or more without creditable drug coverage after turning 65, you accrue a 1% per-month penalty calculated on the national base premium.

5. Still Working at 65? Your Options Explained

If you or your spouse are still actively employed and covered by an employer group health plan at a company with 20 or more employees, your coverage is generally considered creditable. You can delay Part B without penalty until you or your spouse retire. However, if your employer has fewer than 20 employees, Medicare must become your primary payer. Jignasha Patel verifies your group plan status to prevent enrollment missteps.

6. Veterans Benefits (VA) & Medicare Synergy

Many Georgia veterans ask if they should enroll in Medicare if they already have VA healthcare. The answer is almost always yes. VA coverage only works at VA hospitals and clinics. By enrolling in Medicare Part B or a $0 premium Medicare Advantage plan with Part B givebacks, you gain the freedom to receive treatment at private local hospitals in Americus and regional Georgia health centers.
